An application will be e-mailed to you or sent regular postal mail within 7-10 business days of receipt of your application request.  E-mailed applications are sent in Microsoft Word format.

Once you have received your application, please visit these two links, print the forms, and complete them:

 

1.  Virginia State Police - Criminal History Records Check SP-230.  (Adobe Acrobat Reader required.) 

        Get a free download of Adobe Acrobat Reader by clicking the Adobe icon. 

Although this form states you must submit a fee with the request form, there is no charge for this Records Check since it is for a volunteer rescue organization. See Code of Virginia, § 19.2-389, paragraph 11 for further information.

2.   Virginia Division of Motor Vehicles - Driving Record Request  (Adobe Acrobat Reader required.) 

A valid Virginia drivers license is required to operate any PVRS vehicle.  Exceptions to this rule will be those as listed within the Code of Virginia as they relate to full-time college students or members of the Armed Forces.

 

 

Your application MUST be submitted with a completed criminal history records check request form AND a recent copy of your driving record, dated within 30 days of your application submission date.
